What to Bring to Your Appointment

A Little Preparation Goes a Long Way.

Before You Come In

We want your visit to go as smoothly as possible from the moment you arrive. Bringing the right items to your first appointment helps us check you in quickly, determine your sliding fee scale rate, and get your care started without unnecessary delays.

Don’t stress if you can’t pull everything together. Come anyway and we’ll work with what you have.

What to Bring

A Valid Photo ID A driver’s license, state-issued ID, passport, or other government-issued photo identification.
Proof of Income This is how we determine your sliding fee scale rate — what you pay is based on your household income and family size, so we need to verify it. Any of the following will work:
If your income situation is informal, irregular, or you have no income at the moment, please call us before your appointment. We will talk you through what documentation, if any, we can work with.
A List of Your Current Medications Include all prescription medications, over-the-counter medications, vitamins, and supplements you take regularly, along with their dosages. This information is important for your safety during treatment.
Your Medical and Dental History Be prepared to share any significant medical conditions, allergies, or past dental procedures. If you have X-rays or records from a previous dental provider and can obtain them, bring those as well — though they are not required.
Your Primary Care Physician’s Information If you have a primary care doctor, bring their name and contact information. If you don’t currently have one, that’s okay.

What You Don't Need to Bring

You do not need dental insurance. We serve patients regardless of coverage status, and our sliding fee scale is designed specifically for people without traditional dental benefits.
You do not need a referral to schedule as a new patient, though we do welcome patients referred by recovery programs, social service agencies, and community health organizations.

You do not need to have everything perfectly in order. We have worked with patients in every kind of circumstance. If your situation is complicated, just let us know — we will do our best to help.
